Upon reaching retirement age, you can submit a claim for long-term care benefit.
New service! A claim can be completed and sent with all required documents through the volunteers of the Counseling Service to the Elderly over the phone at *9696. The volunteer answering your call will complete the claim for you, and assist you with submitting documents.
Documents to be submitted
Claim for a long-term care benefit form - whether you are living in a retirement home or in an institution, the direction of the establishment must fill in section 9 of the claim form.
If you are hospitalized, you can submit a claim in the framework of "
First Class" through a social worker at the hospital in which you have been hospitalized.
Latest medical documentation - printed medical information involving a digest of the medical file, tests, treatments and medications, or summary of hospitalization which occurred within the last three months.
Confirmations of income - confirmations of income for you and your spouse (except allowances from the NII, retirement pension or any allowance to Holocaust survivors and victims of Nazi persecution), for a period of three months within the last four months.
Power of representation - if the person who signs on and submits the claim is a custodian and not the benefit claimant himself, a copy of the custody order must also be attached to the claim.
Certificate of blindness - a blind individual must attach to the claim a copy of his or her certificate of blindness.

Human response is available from Sunday through Thursday, between 9:00 and 13:30. After operating hours, you can leave us a message on our voicemail and we will revert to you.
After submitting the claim, if you meet the basic conditions of entitlement, your claim will be examined in accordance with medical documentations, and an assessor will schedule with you a phone call to check your functioning level. If you wih him to come over for a home visit - let him know during your phone call.
In specific cases, it might be possible to determine the entitlement based on medical documents alone, without any need for an assessor's verification.
Important information!
If you have reached the age of 90, you can choose to be examined, for your functioning assessment, by a medical doctor specialized in geriatrics, instead of a long-term care assessor from the NII, for more details, click here.
